Much of the recent attention in health reform has focused on the decentralization of authority to sub-provincial levels. However, in most provinces, regions or districts there has been a consolidation of decision making. Many of the previously independent silos of funding for hospitals, community services and agencies have been eliminated.

Incorporating citizens' views into decision making has become an integral part of the management of the health system, whether formally through the mechanism of citizen "governors" on regional or district boards or less formally through public consultation exercises.
