Pass It On! is a series of stories about successful changes to the way healthcare is delivered.
Each story details an initiative that was either motivated or enhanced by evidence – whether observed in a specific project or emerging from scientific literature – and has resulted in better health outcomes for patients. The profiles provide practical ideas that can be adapted and used to inspire change in organizations across Canada.
Pass It On! presents up to four stories on each of the following topics:
Effective practices in healthcare governance
Stories that show how healthcare organizations have structured, designed or used their governance team to improve care and services

Integrating advanced practice nurses
Stories that demonstrate how advanced practice nurses, such as nurse practitioners, have been employed to improve care outcomes

Patients as part of the healthcare team
Stories that provide concrete examples of how organizations – such as hospitals, community health centres, regional health authorities and provincial ministries – have successfully included patients and their families in the teams providing care and service
